TeX Compile Error

When I compile a TeX file (such as "sample-amc.tex"),
an error occured. The error is follow,

!Undefined control sequence.
\fancyhfoffset
               [EOLR]

This is a part of "automultiplechoice.sty",
what should I do?

\fancyhfoffset is defined in the fancyhdr package.
Can you post the LaTeX log file to check if this package is properly loaded, and tell us which fancyhdr version is installed (look at the first lines in the fancyhdr.sty file)?
