Some scanned forms not processed

Hello
I’ve been happily using AMC for some years now, but have hit a puzzling problem this morning.
I have scanned in 3 forms totalling 6 pages which form part of a single PDF document. I have then run this though the automatic capture via the capture tab. The status at the bottom shows that the system identified the 6 pages in the PDF and 3 forms, but then only shows two pages in the Diagnosis list. These are two pages from different forms, not one contiguous form.
When the forms are processed there is no indication that any forms were unrecognised and no errors/warning appear about forms not being recognised.
I’ve attached the debug log. Can anyone suggest a reason/fix?

It appears from your log
file that from the 6 scans, 3 are from page 1/1/60 and three from page 1/2/59, and that the "photocopy" mode is not enabled.
Maybe you wanted to choose the "photocopy" mode?
Without "photocopy" mode, AMC only retains the last scan processed for each page.

Thanks, that's done the trick. I did print separate sheets, but someone must have photocopied some without me knowing!
